Key Responsibilities:
- Develop and implement personalised care plans, reviewing and updating them regularly.
- Shadow new carers, conduct competency checks, and offer ongoing support.
- Provide hands-on care in the community as needed.
- Be the first point of contact for both clients and carers.
- Report regularly to the Homecare Branch Manager on service delivery.
- Maintain the dignity of service users and uphold high standards in their homes.
- Work collaboratively within the care team to ensure a seamless service.
- Commit to professional development through training, meetings, and supervisions.
- Adhere to health and safety policies and procedures.
- Represent the service with professionalism and integrity.
- Handle sensitive information with the utmost confidentiality.
- Promote independence, rights, and choices for service users.
- Efficiently manage rotas, ensuring timely and clear distribution.
- Train and assess staff in safe and effective medication management.
- Conduct care reviews with service users to ensure their needs are fully met.
- Participate in the on-call rota as required.
- Minimum of 2 years’ experience working in a care setting with vulnerable people.
- Willingness to undertake an NVQ or other relevant training.
- Strong empathy and understanding of individual care needs.
- Excellent time management and the ability to work well under pressure.
- Confident in making clear, balanced, and timely decisions.
- Collaborative and proactive approach to team and care planning.
- Commitment to ongoing mandatory and specialist training.
