Our client have an exciting case management opportunity to join their Field Multi-Trauma Team.
They are a leading supplier of injury management and rehabilitation services and they are dedicated to deliver positive outcomes for individuals through their high-quality service. As a member of their team you will provide face to face assessment and case management support to individuals who have been injured or are ill.
Your role will involve conducting face to face initial needs assessments to determine the barriers to recovery and returning to work. Working with all stakeholders, you will develop creative solutions to overcoming those barriers. You will then have the responsibility for the overall management of the case and the outcomes. Depending on your skills, you will have the opportunity to undertake specific assessments that could include Activities of Daily Living Assessments, Worksite Assessments and Ergonomic Assessments.
The role will be home based to cover field assessments in South West.
A comprehensive induction, learning and development and coaching programme will be provided and you will have the opportunity to work with a range of service users and multi-disciplinary colleagues.
Your qualifications for the role:
- A Bachelor degree in Allied Health/Nursing/Rehabilitation Science (ie physiotherapy, occupational therapy, occupational psychology, exercise science).
- Extensive clinical experience in the area of physical and/or psychological injuries.
- Experience in a range of clinical, injury and industry settings.
- Strong analytical and report writing skills.
