Experienced and established Complex Care Administrator to work within the community supporting Continuing Health care.
This role is advertised at a Band 4 AFC and based in different locations in and around Norwich, Norfolk.
You will be directly responsible for providing the administrative support to the Continuing Health care of the patient.
This will involve liaising with internal and external departments that would be responsible for providing care to these patients.
You will provide statistical reports to update on the progress of the patients progress and provide administrative support to the Continuing Healthcare Team.
You will be a strong communicator with the confidence and experience to speak to stakeholders at meetings, via email and on the telephone.
Provide administrative support to plan urgent care packages, structure packages of care specific to the patients medical needs in a timely manner
Prepare case records to present at panel meetings
Good standard of IT skills
